If you want to reinstate your license via mail then provide your full name, date of birth, mailing address, Social Security Number, and phone number.

If you want to reinstate your license in person, you must visit one of the Driver License Reinstatement Centers. The closing time for reinstatements is 4.30 p.m. The cost for a reinstatement license is $65.

Replacement and reinstatement of a Tennessee Driver License
